How to change the name or address with SIA?
All SIA Licence holders must inform the SIA if they change their name or address.
Change of Name
If your have changed your name you must provide SIA with the legal document that made the change (Decree Absolute, Deed Poll document or Marriage Certificate) and complete a 'Change of Personal Details' form or a covering letter containing their old and new signature.
Change of Address
If you have changed your address you must provide the SIA with details of the change, either in a letter or by completing a 'Change of Personal Details' form. You should also enclose atlas one Group B document showing the new address (SIA document checklist provides more information on this).
